COVID-19 Notice for patient
Published & reviewed by Info-Physio: June 6th 2020
The guidelines set out below are advised by our governing body the Chartered Society of Physiotherapy / NHS England / Gov UK.
The following criteria, systems and protocols have been put in place by our clinic (Info-Physio) to help manage the infection, prevention and control of Covid19, to ensure safety in the work place and to protect our patients, staff and the wider community.
We ask patients to work alongside our clinic staff to make sure they have been screened and pronounced "Covid-Fit"
to attend a Physiotherapy appointment. The following are our guidelines on how the process takes place.
• On receiving your appointment request, a member of staff will contact you to complete a "First point of contact screening
form".
•
You will also be asked to complete a medical triage assessment at the same time.
• A decision will then be made in an agreement with yourself if a face to face appointment is justified and warranted, or that of an online appointment is the next step to be taken.
•
If you agree to continuing, you will be offered a virtual online appointment as a first option, in the understanding that we must follow governing body guidelines.
• If a face to face appointment is justified we will then ask you to complete the section on the screening form entitled Covi-19, and directed you to our website and ask you to read mandatory information, that has been made available in accordance with the regulatory governing bodies. We ask that you do view this information as confirmation will be required before your appointment for your own safety and that of others.
